Worker's compensation (Picture) A Worker's Compensation claim may be filed when an employee is injured on the job. If you have been injured on the job and wish to file for Worker's Compensation, you MUST notify your employer of your injury IMMEDIATELY. Patients are encouraged to come in a hour ahead of their proposed appointment in order to fill out our intake paperwork as well as to allow sufficient time to collect all appropriate documentation. The following documentation must be received by us BEFORE you can schedule your first appointment:
 
1 Employee's name, phone number, and address.
2 Worker's Compensation claims adjuster's name and phone number.
3 Worker's Compensation Claim Number.
4 The State in which the injury occurred (We only accept cases which occurred in Virginia).
